1.  The Date  of the recording
  
2.  Description [Sermon, Afternoon Study, Prayer Meeting, Evangelistic Effort, etc]

3.   What would you like....DVD*, WMV Data Disc**, MP3 Data Disc***, or MP3 CD****

4.  If you are a member of the Pico Rivera church, you can pick up the discs at church.

5.  If you are not a member, please give an address you would like to have this sent to.

6.  As for the amounts, please speak to Melanie personally (or through email) individually.
